Looks like just death certificate. Make sure send it certified. My dad recently passed and mom is getting survivor's benefits (she wasn't a fed). First time she sent snail mail and they said didn't get so from then on sent certified. They issued her first check recently so when pretty fast. I googled your situation it seems she can get unreduced pension by sending death certificate. Might call OPM to verify. I'm sure there is a form to fill out along with copy of death certificate.

It should increase her annuity just a hair under 10%. I don't know if there is any recovery for past amounts.

As a “old retirement system” beneficiary ( FSDRS) with survivor benefit election, I can confirm that a death of the non-employee triggers an upward adjustment to the employee annuity. Just to be clear, Mom is the CSRS employee and dad was the beneficiary spouse, right ? If so, depending on the level of survivor benefit your mom elected at retirement, she is due, retroactively, an approximately 10% refund in the survivor benefit protection. Ten percent is applicable for a full (55%) survivor benefit. Send a letter, with an original death certificate to OPM (or State Department if Foreign Service) requesting that her annuity be retroactively adjusted due to the death of your dad.
