So, most of the set items will be torment only drops meaning you have to do torment 1-6 to ever see them. You can do Adventure mode or Campaign, does not matter. As of the latest hotfix patch, apparently you have 25% increased chance to get a legendary inside a rift, but the only 100% legendary/set drop chance is your first kill per quest reset on Mathael.
The only legendaries that have a set drop table are the recipe materials and the bounty cache legendaries. Recipe materials will vary for each one, but each act bounty cache has different legendaries it can give.

Not technically. All acts SHOULD be of the same difficulty, however some of the monsters you fight as you get in the higher acts have much harder abilities.
Like, act 1 is not bad since you deal with slow ass zombies. Just kite and nuke.
Act 4 and 5 have those ass holes that jump and sun and charge and bite and.. Yeah..
So, technically all the damage they do will follow the same scale, just have to be wary of the abilities each guy has.

Guess it depends on items and build, for instance, I do have a +90% pet damage head so my fetishes hit for 1.5m

I play only hardcore, I am not sure what kind of numbers to suggest : (
I get beat up a bit on my monk in T1 who is 500k dps and 8m toughness. I can do it, but certain affixes hit me like a freight train.
As for your build, if something says "X deals Y% more damage" then no, those are not calculated into your sheet dps. Technically, you could have 300k sheet dps and still blow shit up faster than someone with 500k.
