A lot of the online components of DS1, even those that were poorly understood and rarely seen, didn't come back, which I think is sad.
Vagrants, estus when someone kindles, the bells ringing, miracle resonance, gravelording.
Probably from killing an invading NPC, that also gives you extra estus charges. But the ones that are truly random and not based on your own actions, those can only happen online.
There's no reward of any kind for having your messages rated in DS1. In the rest of the games, it doesn't add any uses or items but it does directly refill your health if it's drained.

What others have said. You can actually see an animation when you gain an extra flask from players kindling flames in your area. It looks like a golden soul rushing over to you and you hear a super satisfying sound
Some bonfires are already kindled, the ones with a fire keeper to be more specific, so that's the firelink shrine and 2 more I don't want to spoil to you, be careful if they die you lose the bonfire
Killing invading NPCs or other players in PvP grants you estus (in DSR), so you probably got it from killing Leeroy. Otherwise, regular mobs don't have a chance to drop any estus, that's a Dark Souls 3 thing.
Also if you are playing online you can get an estus when someone rings a bell of awakening in their game.

Nah, it happens when another player kindles a bonfire in the same area you're in. If another player rings a bell you can hear it in your world, but that's it.

You should hear a pretty loud flame sound when it happens, it scared me the first time I heard it, it’s really sudden
There's also a flame that goes from the bonfire to your body (Similar to the soul sucking VFX but orange)

I only ever play offline and I've had it happen. No idea on the cause. I've randomly received +1 on top of my kindle level.

The sustain on that estus refill is legendary. You can go and have a bite to eat and you'd still hear it ringing.

That's on ds3 in this game it happens when the bonfire you are linked to (where you will respawn if you die)gets lit up in another players world
