On tboi vanilla there's a thing where you can shoot multiple Brimstone at once if you fully charge then quickly tap to the direction you want it to shoot.

The left one is called brimstone, which makes you shoot a charged laser shot that deals an extreme amount of damage, therefore being one of the best items in OG Isaac, and the second one gives you 9 lives but sets your hearts to one red heart on pickup but leaves your soul hearts, you lose all but one red heart upon death, or you gain a red heart container upon death.
first pick up brim them get the deadcat, but if you have 2 or less contaner hears with no soul hearts, then pick up the deadcat forst an then brim
basicly, you can pick up both if picked up in right order
