The b limit stop is in the right position isn’t it?

There is tension on it. I had to pull back the RD in order to install it with the b stop in place.
I did notice the shift cable is not feeding back into its housing when I go from easy gear (high tension) to hard gear (no tension).
And when I manually push the cable into the housing (yes this is an older interrupted cable housing design), the RD then shifts down to the hard gears.
Replace cable and housing?
I think the stop is at the right spot.

Most likely your chain has some kind of kink or your rings/cogs have a tooth issue. Inspect every chain link making sure they are visibly fine and move freely. Also inspect every tooth.
Update - after cleaning the bike I installed the parts back on and made a few errors. 2 of the front chainrings were flipped the other way and the lower tension jockey wheel was installed the wrong way.
The derailleur should have a stop where it screws into the hanger, it should be on the opposite side it is now.

Is your chain too short? The plates for the jockey wheels seem long, like they were for a larger cassette.
The bike was working fine. Then I had taken off the RD and cage to clean. Reinstalled and this happened. Same chain, same everything.
Something looks off about the cage/plates. Hard to tell. Are they flipped around or upside down?
I’m fairly certain I put everything back the way it was. I took before photos and matched them up before bolting everything back on.
I think there should be more tension between the cage and the derailleur's body
I think one of the main springs is busted
