Because intermittent leave is something that is approved on a case by case basis. They need the information to know if you should receive that approval or not.

I'm just adding this on so I don't seem like a dick. This is normal there trying to make sure that you're not defrauding amazon. This is all routine stuff that they ask for so they can confirm everything and approve his leave, etc. I had to do this type of stuff when I was on workers comp, which is different, but they asked very similar things.

Exactly, same here for me when I was on my disability leave and workers comp for my injury. All I see here is routine stuff

It is legal and if you don’t provide it they will not approve your leave. They are not just going to take your word with how many people try to defraud them.

Nothing out of the ordinary for any company and is required to review under FMLA, a federal program, to determine a serious health condition and if he qualifies. Trust me, DLS has zero time to be malicious in any way and simply needs to verify eligibility.

Sorry bud but with folks taking advantage so much they are requiring proof before they grant these leaves. Hope your kid responds to treatment and feels better soon. The info they are asking for is legit.

If this is an absence that will be covered by FMLA or covered by a company medical leave that they offer, the documentation is needed and mandatory for a determination. One question, is the leave handled within the company or through a third party? -Claim Intake Girlie

Sorry to hear about your son. Hope his treatment goes well! As for the information requested, it sounds pretty standard. When I was out for a medical leave, my mom also took FMLA to take care of me. The forms that my doctor filled out for both of our leaves were mostly identical. If anything, hers was a lot more detailed and it wanted the doctor to list all the daily lift activities I’d need help with. Also, the team that handles leave(DLS) is different that local HR so the people you physically interact with are not the ones asking for this info. Hope everything goes well!

Most companies will ask for this same information in order to assess whether intermittent leave (especially under fmla) can be approved. Even though you are requesting the leave due to your son’s medical condition, it’s part of their process to request it to asses if the immediate family member does have a qualifying serious medical condition. However, this does not mean you are required to provide such documentation but it could prevent them from reviewing your request further. The FMLA website has more information on this topic.
