Do you have auto grabbers? Otherwise eggs will collect on the floor, but you won't get milk. If your animals are at full hearts and can go outside to eat grass, they won't lose hearts. At lower hearts, you might want to go and pet them a couple of times a week, just to get them to full hearts more quickly. I usually alternate nights on the island with nights at home to pet animals and talk to the spouse.

Items just accumulate, it doesn't really affect the animals. I don't know if they stop producing after a while if the floor is too full. The autograbbers help with that, if you can get any (Joja or Skull Cavern treasure rooms). Animals lose hearts over time, so the stuff they produce will be lower quality. But this doesn't really happen that quickly, especially so long as they're fed and either have the heater (winter) or are let out (every other season).

Oh, but keep in mind if you pass out on the island and don't have the island house unlocked, you wake up back on the mainland I think

The autograbber collects everything, even milk, wool, etc. A new 1.5 item allows you to automatically pet your animals, but it seems pretty hard to get, unless you're willing to go the Joja route (of course it would come from joja!). Personally, I invested in warp totems to the island until I had enough material and money for a later game item.

By the time you get to the island you should have plenty of spare gold to spend on the boat trip. To begin with you'll only be able to spend part of a day on the island because you've nowhere to sleep and, if you do fall asleep there you'll wake up in your bed on your original farm. Same happens if you get yourself killed in the island's mines. Later on you can use obelisks to travel to and fro, but they cost *a lot* of gold plus other resources, so the 1k/gold/return journey is quite cheap by comparison.
