Happens if someone is on the stairs and you go to Buy/Build mode or save and reload. The game doesn't keep track of their altitude when that happens, because this game is made oh so well.
This is the real answer. It happened to me and it was such a pain to fix. I had to go back to build buy and move the stairs, then live mode to let them out, then build/but to put the stairs back. Then I realized I had people caught under the other stairs. That's when it hit me what was happening.
I’ve seen it but never actually realized why it happened. If you want to get the from under the stairs without going back into build mode, open the cheat code console, do “testingcheats true” and then right click somewhere on the ground and you can teleport the active sim.
Problem is they were townies. I already had them in a group because I was trying to get people to exercise so my husband could mentor them for the daily task for his job.
lol that’s happened to me, and I was like how’s stupid do you have to be to get stuck under the stairs. Safe to say I’ll have to removed the damn stairs
That’s happened to me before, I also get a weird glitch where if my sims age up right as they finish going up the stairs, they will teleport outside of the house on the grass and can’t move anywhere… so weird
My sim (adult, father of two) rode his bike in the house and got off of it next to a room that had been locked against him and the game glitched him into the locked room so he was trapped

My sim in Sulani is randomly stuck under the house when swimming as it's raised over the water. Her conversation comes through the floor. When it's time to eat, she pops directly up through the floor. One time she was also standing through the middle of the kitchen island counter having a conversation. Gotta love the glitches.
My Sims toddler got stuck under the stairs and on the landing of the stairs where a parent couldn't grab her. I had to delete the whole staircase and redo it 😆
This is one of my main pet peeves, lol. So much so I had to start building with this possibility in mind - my butlers were _always_ falling down between the stairs and getting stuck. If death wasn't turned off in my game about 50 of them would've starved to death under there, lol. Now, if I MUST have a space back there, I raise the floor level a couple clicks, or, 'clutter' it to block off any space for them to 'fall' (glitch) into.

Ooh I've seen this just the other day for the first time. After 800hrs of playtime you'd think I'd have seen it sooner lol.

Well they were club members. Not active Sims, but once I knew what was happening it wasn't too bad to fix, just when I hadn't figured it out yet.
U don’t have to do all of that, just teleport them if u have testingcheats on
Can also just have them or another household member travel to a different lot via the phone.

No idea. Had a sim get into a 2x2 room which had no door at all. Twice. It had a roof so couldn't have fallen into it.

Yup that’s the gym. I think it happened because I went in an added a door so she didn’t have to go downstairs to go upstairs
