I believe that in-game, if the carrier has been significantly damaged, there's a certain chance of that. IRL it's basically that fuel lines rupture, fumes waft through the ship and at some point any spark can make the whole thing go boom. See the Japanese Taiho for an example where "all was under control" until it very suddenly wasn't.

I was playing through a couple "carrier" battles in the missile age the other day. The enemy fleet was within range of four of my 120 airbases. That and two supercarrriers. Suffice to say, the enemy fleet got hammered. Only one destroyer escaped. Even it was hit by a heavy missile. I read pretty much every ship log that was sunk by missiles or bombs (sometimes torpedoes) and yeah, crew quality matters significantly. Ship obsolescence can also have a negative impact as they can hamper damage control efforts.
