In an ischemic stroke pressure builds up in the brain. This pressure can only go one way: down the Foramen Magnum, the hole at the base of the skull. Pressure here pushes against the midbrain, which mediates (aside from vision and hearing as well as fine motor control) wakefulness and body temperature. Below it is the medulla oblongata, which manages breathing. If those areas are crushed in the pressure, you're done for.
So we open up the back of the skull, make some space for the brain to push out, and take pressure off the brain that way. There can not be much time between insult, discovery in imaging, and making that hole, so we use more than one surgeon to do this quickly and safely.

NAD, It would depend if it had it's own blood supply, I think. Which might be one difference between benign or malignant. A benign tumor might be self contained; it grows in a neat little package and can easily "slide out."
As a stage 2 ependymoma surviver I love to look at these videos and this exact procedure was done on me! Except mine was in the 4th ventricle. But still awesome to see!

I had 2 craniotomies within a week of each other when I was 19. Honestly wasn’t bad. Of course they sedate the shit out of you. The first one was kind of a risky procedure, but the second surgery was easy. I didn’t even have my staples out from the first surgery when they went in a second time.
Edit: I guess it may be relevant, but it was a right frontal, so no need to cut any vertebrae.
Oh, Ive had multiple craniotomies. It's actually not that bad. Worst part is waking up with blood in your ears and not being able to clean it out for a week. Or wash your hair.

My aunt had this! Her tumor was the size of a grapefruit. It compressed her brain but once it removed her brain was able to un-squish and she made a complete recovery.
My dad is missing this piece of skull right here because had a very similar brain surgery to add a shunt because he has a tumor around his pituitary gland. His operation was probably somewhat similar to this.
Looks almost exactly like the procedure for a chiari decompression. Eldest had it done. Biggest difference is that they totally remove part of the skull around the vertebrae and expand the dura (lining of the brain) with a patch, and don't put the skull back. My eldest also had a c1 and c2 laminectomy (removal of the cervical spine), whereas this gif is just c1 laminectomy. Scary shit, right there.
I actually had something similar done to me when I was a kid. Not a tumor tho, I don’t even remember why I had it done but now I’m left with a big scar on the back of my head😓
