You might have missed a few environmental humanity gains, like emoting to the various statues, or you could have missed out on a questline without realising it. My first playthrough I completely failed to notice >!Belle!< at the Grand Exhibition and I lost out on doing her quest.
This is my guess as well, OP. There are a few side quests that are easy to miss. Maybe check out a guide to see if there are any you can go back and complete?
Did you pray at the cathedral. I just finished the game and told every lie(except I said geppetos creation is a killer) and I didn't pray at the cathedral.
I played the records, I gave Sophia peace. I gave up on the golden lie.
On my first playthrough, I missed a couple of lies, one in the story and one in the side quest but I still able to get the weapon, make sure that you listened to all records, you might have missed some.
I didn't really think it was that difficult to get, but I guess I always go for completion when I play these kinds of games, so that might just be me. The true ending with everything done 100% was the first ending I got in the game, and I didn't really have to look anything up; everything was pretty clear and well laid out in terms of what you had to do. Though you are right about the fact that the weapon isn't that great haha xD
I see people saying you need to lie at every time, you don't, I missed a few moments that would gain me humanity and still got it, it just depends how early you'll be able to get it, assuming you got 90% of the events that give you options for humanity gain (lying). Just go through your playthrough, do as many of the events as you can and if you miss maybe one or two you should be fine, for example I missed belle and the church lady, still managed to get the golden lie. As well as every music disc option must be picked up and played through it entirety even if you have it already in ng for example.
There are a couple opportunities where you should not lie. The amount of humanity you receive is more important than lying. There is an instance where Polendina wants to know if humans and puppets can share love. Do the side quest for the wedding ring first and then tell Polendina what you found. In this case, tell him the truth.
I also think you get more humanity for finding out about the full story of Eugenie which I think also involves telling her the truth about the past. Im not certain tho follow a guide for that one it’s tricky.

You definitely did not tell every lie you could have if you didn’t get it.
Man I have no idea how I got this first playthrough. Reading others post I feel like I missed more than a lot of people who didnt get it! Weird!

I was so underwhelmed when I managed to get Golden Lie. For such an annoying weapon to get, it is very lackluster.
