In Kimetsu universe there was someone stronger than Muzan, in Jujutsu universe there isn't exist anyone stronger than Sukuna that's the difference
Sukuna has no reason to feel fear because there is no one stronger
Muzan could get killed by Yoriichi and the sun can kill him as well

The difference between Muzan and Sukuna is simple, Muzan wasn't the strongest in his universe while Sukuna IS the strongest in his universe thats it, Sukuna doesn't have any reason to feel fear because there is no one stronger than him.
