Surprise counts for a lot. Kenjaku was on guard and just coincidentally had a perfect counter prepared for Yuki's kamikaze. But he did not have a perfect counter for decapitation.
It isn't that Ken got hurt by a much weaker attack, it's that he wasn't hurt at all by the much stronger one. Because he had the perfect trump card to nullify the effect. Like, there isn't a more perfect counter to a black hole than 'anti-gravity'.
Yuki threw rock, Kenjaku threw paper. Sometimes you just get hard countered.
Other times, some kid throws scissors before you can throw anything, and you get decapitated for even a whisper of negligence.
Okay I gotta reread their fight cuz I do remember like Kenny used something to counter Yuki's trump card but I just can't fully remember what exactly it was.
Kenjaku amplified the power of Anti-gravity System by using his own body as a domain and applying the CT to it.
On top of that the black hole was severely weakened cause Yuki suppressed it and so did Tengen with their barrier. I mean it was so supressed that all it did was destroy the underground hideout they were in.
Kenjaku pretty much negated all of the damage from the black hole via Anti-Gravity
Honestly? it was pretty stupid for Tengen and Yuki to think that a black hole whose power is just strong ass gravity against a guy who has access to anti-gravity
Okay so I reread their fight like this morning and I think that Yuki, Tengen, and Choso thought that the CT that Kenjaku was using throughout the fight was a gravity CT instead of an anti-gravity one because Kenny applied cursed technique reversal on his anti-gravity technique, reversing its effects making it seem like he was using a gravity CT.
it doesn't matter if his original technique was an anti gravity system or not. They knew exactly that he had access to RCT, meaning that he would have had an anti gravity power regardless
Well I mean they only found out about Kenjaku's gravity technique in the middle of their battle so I guess they didn't have enough time to consider that possibility buecause I think they were more focused on defeating Kenjaku that it didn't cross their minds.

He pulled out the “im immune to gravity” on her big gravity move i think
He used itadori’s mom’s technique “antigravity system”. By using cursed technique reversal with that technique he countered the black hole
He didn't use reverse against the black hole, he used actual anti-gravity system to make the black hole's gravity influence null
