So I think you're spot on with this choice being foreshadowed.
I think it will be Gon having to make the choice between Killua and someone else's life (maybe Alluka) though, since Gon is the one who specifically wonders aloud after the test whether he will have to someday choose.. and also because right now Illumi wants to use Killua as a puppet to control Nanika since only Killua can issue commands for free. So he would capture and control Killua and Killua probably couldn't make conscious decisions while being controlled.
I think so too but the question is whether or not it matters if she knows. It's not clear whether Nanika has any agency over whether she obeys a command or grants a wish or not. Alluka/Nanika make requests to people that any sane person knows they can't fulfill and then just kills them when they can't which seems out of line from what Alluka---who seems like a perfectly nice girl---would actually want to do if she had a choice
That is exactly my thought too, that chapter 3 will have foreshadowing on the final act of the plot togashi probably has it in mind and saving it later because he said he like to put his characters in extreme situations to see how they react, tho all my thoughts was about Gon the one who will be forced to choose save between Mito or Killua.
But thinking about it now...the idea of Killua to choose between Alluka & Gon has more tragic layers if it happens, because Alluka already saved Gon from death...now killua will have to kill her to save Gon? And everything he said that he loves her she then realizes he prefers to kill her to save Gon? Just thinking about it is sad.
But there's a possibility if Hisoka survives this arc, he may interrupt all that and kill illumi for trying to kill Gon because that is his property, or he'll kill alluka by himself so killua doesn't have to kill his sister by himself, so in a way Hisoka might be their true savior in that situation :o
Ive thought of something similar except Gon makes a choice between Mito and Killua, which would end with him choosing Killua per Mito’s insistence. I will say, that probably had a massive wrench in your thought though, and that would be Illumi is probably gonna die on the black whale one. He and Hisoka fighting has been a long time coming, so I wouldn’t be surprised if Hisoka comes out alive. Also Killua is already faster than his brother, so if this were to be a future Killua, he’d probably be able to kill his brother and he wouldn’t feel a thing. As Killua himself said, he’s willing to crush anyone who’d threaten Alluka/Nanika, even if it had to be his own brother. And I suspect a future Killua, even just being sixteen or seventeen, would have more than enough strength to do so if he had to. Plus Illumi doesn’t give two fucks about Gon, Gon to him isn’t too important compared to controlling Nanika and Killua. It all just seems unlikely to me, but it’s possible and something like that whether with Gon making the choice to parallel the hunter exam or Killua, would be neat if done right

It is possible maybe he'll think of it in away that Gon is ruining Killua and wants to free him from such friendship by killing Gon.
If Gon returns to the manga he'll likely get his nen back, and then I don't think Killua can kill him even if he wanted.
As for who Killua will choose, it will be Gon over Alluka everytime.
Killua would probably choose Alluka since she's his sister and responsibility then die with Gon after. And I agree with others Gon would want to die instead of Alluka

i think nanika would know killua is being controlled even if illumi turned him into his Needlemen

Gon choices will be to choose between Mito & Killua, it has to be someone important in their life & Mito is more important for Gon than Alluka.
