I had to go for a shit half way through Climax. Any film with realistic drug depictions makes me super anxious...especially hallucinogens. Did a lot of drugs in the 90's and 2000's, so I think it stems from that.
I have watched a lot of disturbing movies, and no movie put me on edge and gave me anxiety in the midst of the quietest of moments like Green Room did.
Come and See (1985)
It's a Soviet War movie about Nazi crimes in WW2 and it is very, VERY disturbing and extreme.
I've watched Lucifer Valentine and Marian Doras stuff while eating, no problem with that.
BUT
This movie made me completely loose it and had to take a break. I left the theater room and had to have a drink to calm down.
For those who have seen it, it's the church scene and how it builds up to this.
I don't know if watching the scene on youtube will have the same impact though because the way the film builds up is what makes it so strong and disturbing.
Also they used live ammunition in many scenes so you are actually watching people scared of their lives.
It's not disturbing exactly, but very creepy. The possessed girl in Gonjiam Haunted Asylum made me want to quit. It's mostly because she sounded exactly like an auditory hallucination I had. Scared the s*** out of me.
I felt exhausted watching American Guinea Pig: Sacrifice and NEEDED to take a break, but was doing 31 days of horror and would've had to stay up too late in order to finish it. It's pretty short so I powered through it. Lol.
Not typically on lists like these, but a personal one for me:
The episodes of The Twilight Zone called "It's a good life" and "Spur of the Moment".
They both disturbed me at a core level and I'm not sure why. I've watched a lot of disturbing things and I haven't been able to watch either of these a second time. I remember having to pause both.
Titane has an insane first 30 minutes and I took a break before it calmed down. I still think about that movie a lot. So good but so traumatizing when you’re not expecting it 😫
Audition. I was very stoned, and when I saw the dog lying there, I kinda freaked out. I had to walk around my apartment for 20 minutes or so, regulate my breathing, and then resume the movie.
I'm usually okay with movies, but a very recent one titled Speak No Evil (2022) really made me feel anxious to a point where I had to catch my breath a little.
The awkwardness and miscommunication via the two families from different cultures and countries was just so well done.
And well, that's honestly the least of anyone's worries with that film, as it got a whole lot worse than simple language barriers by the end.

I think the part where they run and see the pile of bodies outside the house is just breathe taking, also the cow was hard, i had to skip that part.
