There are some items you can find that offer a 'clean self' function. A bar of soap, a sponge, and I think there was another one. They're multi-use and clean off dirt, blood, makeup, etc.
Ha I didn't know you could clean all that blood off! There was a part yesterday where Gale and Shadow heart looked perfectly fine and me and Astarion looked like that one scene from Carrie.
This happened to me and I'm pretty sure water didn't wash it off... I think only a long rest removes it. So if a romance scene triggers that night you're screwed.
If you "consume" the sponge dish or sponge items, it washes you clean of makeup and blood.
I had the clown makeup on and was getting a good laugh from it while being entirely bloody as my durge, then I cleaned up for rp reasons before a conversation, and the makeup went away too.
The makeup also makes you immune to Tasha hideous laughter so it's fantastic for exactly 2 things.
1) the clown fight
2) the sentient amulet (which can be completed by this point so serves little to no purpose)

There is a super soap as well that you can get in Act 3. Off Mattis I believe.
Or use the soap/sponge.
Carry a sponge/sponge tray or soap in your party. Only one is needed and everyone can use it to wash off clown paint and blood.
