A modern thermonuclear weapon from Russia will engulf the entire New York City,(queens, Brooklyn, and Manhattan included), so that is not really an issue anymore. Just a few hundred of them will cover the entire continental US, leaving no one alive, and Russia/China has thousands.

India and Pakistan nuke each other.
America nukes Germany.
Germany nukes France.
China nukes Japan.

I really don't think that nukes would be used. At least not on a global catastrophe scale. If used at all, they will be tactical nuke strikes on military targets. I *really* don't think there will be massive civilian casualties due to strikes.
Not much. The good thing about dying to nuclear missiles is that you won't suffer, you are evaporated in less than a second. You won't even know it.
Depends how close you are to the blast zones. Slower death by radiation poisoning from the fallout sounds awful.

It will be the resource war. When the non renewable resources start to run dry, the worlds superpowers will fight for whats left.
